WHAT WE NEED.

  • Active US DOT and MC numbers, plus an EIN / W-9.
  • A legible copy of your operating authority.
  • Owner and driver CDLs.
  • A current COI — limits below — with us added as additional insured before your first load.

INSURANCE MINIMUMS.

  • GENERAL LIABILITY$1,000,000 / occurrence
  • AUTO LIABILITY$1,000,000 / accident
  • MOTOR TRUCK CARGO$100,000 / incident
  • DEDUCTIBLE$10,000 or less

Cargo coverage must include theft, fire, and hijacking, and must not exclude employee dishonesty. ACH direct-deposit is set up separately and securely after approval — never enter bank details on this form.
